KSHSAA Pre-Participation Physical Evaluation

TO PARTICIPATE IN SPORTS!
This is only the physical portion of the requirements.
To be eligible to participate, you must complete the online registration and The Railer Way Code of Conduct requirements.
The physical must be dated after May 1st of the spring preceding the school year in which the athlete will be participating in sports.
Note, before you leave the physician's office, please confirm that the athlete has been cleared/not cleared and that the physician has sign and dated the form in two places.
